If you become disabled while covered under the Active Cement
Masons Plan, you may be eligible for “disability hours credit”.
Disability hours credit protects you and your family from the
loss of health and welfare coverage in the event you are unable to work. The Plan defines “disabled” as being unable
to perform any and every duty in connection with your occupation.
To be eligible, you must have at least 300 hours in your hour bank at the
onset of your disability.
If you are eligible, the health and welfare plan grants Disability Hours
Credit of 8 hours per day, 40 hours per week. Disability Credit will not be granted for more than six months during any
consecutive 12-month period. (For the actual text governing Disability Credit, refer to Article II., Section 2.a.(2) of the
Active Cement Masons Plan.)

Certification form by contacting the Fund Office, visiting your Local Union or receive the form online here.
